Fill out the form below. If you are registering more than one team, after you click "Submit" you will have the opportunity to complete another form.
Your registration is not considered complete until we have received payment ($250 per team). You can pay via credit card/PayPal, or make out a check to "Hopkinton Basketball Association" and mail it to: HBA, P.O. Box 336, Hopkinton, MA 01748.
